Gather the relevant materials below and follow the outlined steps.

- Confirm that the UW J Visa Request Form and all supporting documentation contain consistent information (e.g., appointment title, proposed dates of sponsorship, activities, and salary/funding)
- Return as one complete packet to International Scholars Operations (ISO) by one of the following methods:
- By campus mail to campus mailbox 351245
- As one complete PDF by replying to the email sent with the approved visa request
- As one complete PDF uploaded to the visa request using the “Manage Files” function
All documentation must be legible, single-sided photocopies or scans of original documents on 8.5″ x 11″ paper. Original (wet) signatures are not required. Do not use staples.
For all requests:
- The signed J Visa Request form
- The offer, invitation, or reappointment letter describing the terms of the appointment
- Evidence that the scholar meets the J-1 funding requirements
- A letter attesting to the amount of patient care, if any, for foreign medical graduates, with original signature
For initial requests:
- A copy of the biographic page of the scholar’s passport
- Copies of biographic pages for all dependents coming to the U.S. in J-2 status, if any
- Evidence that the scholar has adequate fluency in English
For change of unit and/or title and extension requests:
- A new Insurance Compliance Form from the scholar
- If a passport has expired since the first DS-2019 was issued, a copy of the biographic page of the scholar’s new passport and/or of any J-2 dependent’s new passport
For transfer requests:
- A copy of the biographic page of the scholar’s passport
- Evidence that the scholar has adequate fluency in English
- Copies of all previous DS-2019 forms from the scholar
- Contact information (name, e-mail, and phone number) for a Responsible Officer or Alternate Responsible Officer at the scholar’s current J-1 program
