Position Description

The Division of Metabolism, Endocrinology and Nutrition, Department of Medicine, University of Washington is accepting applications for three (3) Advanced Practice Providers (APP) at the rank of Teaching Associate.  The APP will provide consultative specialty diabetes care to patients in coordination with Endocrinologists, certified diabetes educators, and ancillary support staff at the University of Washington Medicine Diabetes Institute (UWMDI). The APP is responsible for providing patient care independently and collaboratively with appropriate privileges, including initial and ongoing assessment, diagnosis and treatment of patient’s medical, physical and psychosocial status.

  1. Provides timely initial and ongoing patient assessment for diabetes management.
  2. Participates in the assessment, planning, care implementation and evaluation of the hyperglycemic patient requiring urgent outpatient care.
  3. Participating in the assessment, planning, care implementation, and evaluation of established patients in the UWMDI.
  4. Attends UWMDI Service meetings with UWMDI faculty and support staff.
  5. Collaborates with the UWMDI supervising physician, the UWMDI certified diabetes RN’s, the UWMDI certified diabetes educator RD’s, the UWMDI PharmD’s, and other providers to ensure coordination and continuity of diabetes care across settings and over time.
  6. Attends appropriate clinical meetings to serve as a resource for staff and providers.
  7. Facilitates collaboration with referring care providers to ensure continuity of care. 
  8. Provides patient and/or family with health education, counseling and referrals to other health professionals and community resources as appropriate.
  9. Participates as a key member of the interdisciplinary team through the development of collaborative and innovative practices and evaluation of patient care outcomes.
  10. Evaluates quality outcomes for the UWMDI through participation in the development of quality indicators.  

University of Washington Medicine Diabetes Institute (UWMDI) APP Duties

  1. The UWMDI APP maintains excellent interpersonal skills to execute the UWMC Best Practice Model.
    • Establishes a therapeutic relationship by imparting a commitment to care for the patient and family through the acute illness and beyond.
    • Connects with the patient and family by eliciting the patient/family perspective and validating the value of that perspective using strong empathic listening skills.
    • Communicates a sense of caring and openness.
    • Communicates effectively with patients and families, appreciating cultural and socioeconomic disparities.
  2. The UWMDI APP provides clinical consultation in all the domains of Glycemic Control Consult Service.
    • Patient/family/provider communication about disease and treatment and how it relates to patient values, priorities, and goals of care across the spectrum of care.
    • Articulates recommendations to colleagues with appropriate and specific language in accordance with current standards of practice, develops individualized goals in accordance with shared outcomes by the primary service.
  3. The UWMDI APP collaborates with the primary team.
    • Makes individualized Glycemic Control Consult Service recommendations
    • Contributes to the plan of care.
    • Prescribes medications in accordance with the primary service.
    • Orders diagnostics/treatments as needed after collaboration with the primary team.
    • Promotes a collaborative approach to decision making.
    • Participates in discharge planning meetings to identify and propose solutions for barriers to discharge.

University of Washington faculty engage in teaching, research and service. This position is a full-time, 12-month appointment (July 1 - June 30 service period) renewed annually with an anticipated start date of December 1, 2021 or after.

Qualifications

  1. Current and valid Washington state ARNP or PA Licensure.
  2. Graduate Degree from an accredited Nurse Practitioner Program or PA-C Program or foreign equivalent.
  3. Nationally certified APP with DEA or foreign equivalent
  4. Proven excellence working in an interdisciplinary collaborative team environment.
  5. Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with hyperglycemia.
  6. Training and experience in Diabetes Care and inpatient Glycemic Control management preferred but not required.
  7. Certification as an Advanced Diabetes Management ARNP or PA and critical care experience preferred
