Position Details
Position Description
The palliative care APP would be faculty in the University of Washington School of Medicine Department of Family Medicine and a clinical member of the UW Medicine Palliative Care service. The APP would be clinically active at the UWMC-NW campus with the palliative care consultative and primary team providing palliative care consultation to medical teams, patients and families, nursing social services, and spiritual care staff. This individual will also be responsible for independently admitting, discharging, and managing comfort care and inpatient hospice patients on the primary palliative care service. Responsible for providing palliative care independently and collaboratively with appropriate privileges, including initial and ongoing assessment, diagnosis, and treatment of patient’s medical, physical, and psychosocial status.
STANDARDS OF PRACTICE AND PERFORMANCE
- Provides timely initial patient and family assessment for palliative care consults.
- Facilitates the palliative care consultation and palliative care team in collaboration with the treating medical team.
- Participates in the assessment, planning, implementation, and evaluation of palliative care interventions including acute and chronic pain and symptom management.
- Attends appropriate clinical meetings to serve as a resource for staff and to identify potential PCCS patients.
- Facilitates collaboration with the PCCS at FHCC to insure continuity of care and coordinate shared UWMC-NW/FHCC PCCS meetings as needed.
- Provides patient and/or family with health education, counseling and referrals to other health professionals and community resources as appropriate.
- Participates as a key member of the interdisciplinary team through the development of collaborative and innovative practices and evaluation of patient care outcomes.
- Evaluates quality outcomes for the PCCS through participation in the development of palliative care quality indicators.
Palliative Care Nurse Practitioner/ PA Duties
1) The Palliative Care ARNP/PA maintains excellent interpersonal skills using narrative medicine to execute the UWMC-NW Palliative Care Best Practice Model.
a) Establishing a therapeutic relationship by imparting a commitment to care for the patient and family through the life-limiting illness and beyond
b) Connecting with the patient and family by eliciting the patient/family perspective and validating the value of that perspective using strong empathic listening skills
c) Communicating a sense of caring and openness
d) Describing the concept that “good medical care” is a combination of excellent biomedical treatment with the patient’s ability to continue to live a life that has meaning and makes sense to them
e) Mindfully eliciting the patient’s story, listening for cues and interspersing probing and clarifying questions into the narrative.
f) Assist the patient and family in clarifying their values and goals for living the best life possible and helping them understand what medical care can support these goals. Stress what medical treatments are consistent with supporting these shared goals/decisions and which treatments are inconsistent with these shared/decisions goals
2) The Palliative Care ARNP/PA provides clinical consultation in all the domains of palliative care.
a) patient/family/provider communication about disease and treatment and how it relates to patient values and priorities,
b) goals of care across the spectrum from aggressive/curative, disease control/life extension, to comfort care/hospice
c) pain and symptom management,
d) end-of-life care planning from simple resuscitation clarification or preferred location of care (home/hospital/SNF), to complex patient/family or patient/provider mediation or withdrawal of life support
e) Participates in infrequent call as needed for the Palliative Care Consultation Service
3) The Palliative Care Nurse Practitioner collaborates with the primary team.
a) Makes individualized palliative care recommendations
b) Contributes to the plan of care
c) Prescribes medications to the extent delegated by the primary team
d) Orders diagnostics / treatments as needed after collaboration with the primary team
e) Promotes a collaborative approach to decision making
4) The Palliative Care ARNP/PA may engage with educating Fellows, Residents, UW SOM students, ARNP & PA students, as well as UWMC-NW staff, on all aspects of palliative care.
a) Participates as faculty in formal palliative care classes through the Family Medicine Department
b) Assists in training of medical students and residents and fellows on the palliative care team as well as on the primary medical teams
c) Precepts students individually
d) Participates in Nursing Education
5) The Palliative Care Nurse Practitioner assists in palliative care program development develops innovative policies, protocols, and procedures using an evidence based approach to improve quality, safety, and value of palliative care
a) Participates with UWMC-NW colleagues on pain and symptom management protocol development
b) Participates with UWMC-NW colleagues on palliative care program development for specific populations
c) Participates with interagency colleagues on palliative care program development across inpatient, outpatient, and community agencies.
This is a Full Time 1.0 FTE position. The base salary range for this position will be $15,000 - $16,667 per month ($180,000-$200,000 annually) commensurate with experience and qualifications, or as mandated by a U.S. Department of Labor prevailing wage determination.
Qualifications
Minimum Qualifications:
- Current and valid licensure as an ARNP or licensure as a PA in Washington State
- Graduate Degree from an accredited Nurse Practitioner Program or PA program
- Nationally certified APP/PA with DEA
- Minimum of 6 months experience working in Palliative Care/Hospice
- Proven excellence working in a interdisciplinary collaborative team environment
- Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with life-limiting illness.
Preferred Qualifications:
- Training and experience in pain and symptom management preferred.
- Board certification as an advanced practice hospice and palliative care nurse or PA by a national certifying body.
