Title:  Mechanical Circulatory Support (MCS), Advanced Heart Failure (AHF) and Cardiac Transplant Advanced Practice Provider (APP), Adult Registered Nurse Practitioner (ARNP) or Physician Assistant (PA)

Faculty Appointment:  Full Time (1.0 FTE) Teaching Associate/APP position in the Division of Cardiology.  This is an annual appointment that aligns with a 12-month service period (July 1-June 30) and may be reappointed to subsequent annual terms.  Faculty with 12-month service periods are paid for 11 months of service over a 12-month period (July-June), meaning the equivalent of one month is available for paid time off. Anticipated start date will be September 1, 2023.

Position Overview: We are seeking a highly reliable and flexible MCS APP with proven excellence working in an interdisciplinary collaborative team environment for our Mechanical Circulatory Support/Advanced Heart Failure and Transplantation Team. Following a training period of 30 – 90 days (depending on background, previous training and experience), the candidate will independently and collaboratively provide Inpatient and Outpatient coverage at their highest scope of practice.  This service currently covers patients 5 days/week (M-F, no weekends or holidays) and shift expectations include the following: 10-hour shifts, 173 shifts/year.

Job Duties and Responsibilities:

  • Participate as a member of the MCS inter-professional team that provides care to patients with advanced heart failure, mechanical circulatory support (temporary and durable devices) and cardiac transplant across the continuum of care and provide care in both the Inpatient and Outpatient settings.
    • Inpatient Responsibilities include:
      • Provide consultative care for patients cared for by CT ICU, CCU, and floor Cardiology services
      • Pre-round on patients prior to interdisciplinary rounds.  Pre-rounding involves physically seeing patients, especially those who may need urgent attention to execute the plan of the day with the most accurate data
      • Prioritize base upon pre-round assessments the order of rounds, based on patient acuity
      • Consultative care to include obtaining a comprehensive history, performing a thorough physical examination, developing a differential diagnosis incorporating history, exam findings, and diagnostic evaluation
      • Present patient on rounds and work with members of the team to generate a plan of care
      • Write timely and accurate notes per UW Division of Cardiology Documentation Standards
      • Attend afternoon briefs
      • Request and follow-up on consultations with primary services
      • Initiate transitions of care and coordinate transfer to other services as part of the discharge planning process
      • Lead, monitor, and reinforce practice guidelines and promote the use of clinical care maps and standardized protocols
      • Educate staff, fellows, other APPs, patients, and families
      • Participate in care conferences and family meetings
    • Outpatient Responsibilities include:
      • Collaborate with an inter-professional team including APPs, cardiologists, nurses, MAs and clinic schedulers to provide high quality and safe, patient- and family-centered care in a premier institution
      • Independently and collaboratively provide comprehensive care of up to 6 general cardiology, urgent add-on, or post-hospitalization discharge follow up patients in each of eight 4-hour blocks weekly (based on 1.0 FTE) in the outpatient setting
      • Diagnose, treat, and manage acute and chronic heart failure, HTN, medical issues directly related to cardiovascular disease, arrythmia, and perform and interpret VAD (Ventricular Assist Device) interrogation, provide troubleshooting and VAD management.  Order and interpret pertinent laboratory and diagnostic imaging in a timely manner
      • Prescribe and titrate diagnostic, pharmacologic and therapeutic interventions
      • Educate team members including physicians, nurses, MAs, patients and families on Cardiology diseases and management
      • Document per Heart Institute Standards via EPIC
  • Communicate in a professional, timely and respectful manner with all members of the health care team whether through verbal, email, secure text, or EPIC communication
  • Report critical changes in a timely manner
  • Promote and enhance communication, education and mentorship to patients, family members, and members of the interdisciplinary team
  • Acquire, maintain, and apply current knowledge of the practice of advanced heart failure, mechanical circulatory support, and heart transplantation.
  • Participate in quality improvement projects and process improvement activities, and support the Division and broader UW education, research, and quality initiatives 
  • Attend Heart Transplant Conference on Tuesday and Thursday morning, MCS operations meeting on Thursday afternoon and Cardiology Grand Rounds on Friday morning, monthly Cardiology APP meetings, MCS Faculty meeting, and quarterly Faculty meeting)
  • Collaborate with physician colleagues in identifying patients for clinical trials
  • Provide accurate and timely order entry via EPIC CPOE
  • Follow UWMC Professional Behavior Policy and Patients are First Guidelines

 

Benefits:

  • The base salary range for this position will be $9,167 - $14,334 per month ($110,004 - $172,008 per year), commensurate with years of experience, discipline, and qualifications, or as mandated by a U.S. Department of Labor prevailing wage determination. Other compensation associated with this position may or may not include Temporary Pay Supplements or Administrative Supplement.

Qualifications

Required Qualifications:

  • Current licensure as an ARNP or PA-C in Washington State
  • Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited Physician Assistant program, or foreign equivalent.
  • Nationally certified ARNP/PA with DEA
  • Highly reliable, flexible provider with proven excellence working in an interdisciplinary collaborative team environment  
  • Excellent interpersonal skills, leadership skills, and clinical expertise in the care of patients with chronic and acute cardiovascular illnesses.
  • Basic computer knowledge and command of basic personal computer skills 

 

Ideal Qualifications

  • Training and experience in cardiology, specifically heart failure management preferred
  • Mechanical Circulatory Support experience
  • ACLS Certification
