DescriptionAdult Congenital Heart Disease (ACHD) service at UWMC was one of the first five nationally accredited ACHD programs in the US. Advanced Practice Providers (APPs – ARNPs and PAs) are integral and essential to the comprehensive outpatient care of this patient population.  APPs collaborate with ACHD physicians, surgeons, nurses and clinic staff in providing the highest level of caring for this patient population. The ACHD APPs independently see patients in the outpatient clinical setting and seek input and consultation from patient primary ACHD cardiologist and/or surgeon as needed. In addition, the APP will assist with the supervision of cardiopulmonary exercise testing – critical study to evaluate the functional status of ACHD patients.  The APP role will likewise expand to include inpatient ACHD consultation.APPs support a model of care delivery that prioritizes rapid access and patient care needs.  APPs share the responsibility of caring for patients with ACHD with their physician and surgeon colleagues and do not carry specific patient panels.  APPs are supported in their practice by a team of ACHD RNs, patient care coordinators, and medical assistants.  As APPs support the entire patient cohort, the APP will collaborate with the ACHD RN, assigned to the patient, in orchestrating and following up the plan of care.
